VALLEY COMPUTER SERVICES DSL CONTRACT
This contract governs DSL service provided to customers by
Valley Computer Services.
Description of Charges
Customer agrees to pay $__________ per month for DSL service provided to
customer by Valley Computer Services. Any equipment provided
by Valley Computer Services remains the property of Valley Computer Services
and must be returned at the end of service.
This signed contract must be returned with the first month service
fee and any activation fees before the order will be placed.
Customer may upgrade to a higher rate DSL plan during the term of this
contract for a one-time fee of $60.00.
Customer may switch DSL service to a new phone number or address during
the term of this contract for a one-time fee of $60.00.
Minimum Service Period
Customer agrees to maintain DSL service through Valley Computer Services
for a period no less than 12 months from the effective date of this
agreement. For new DSL accounts, this agreement is effective when the
DSL service is installed. For DSL accounts existing prior to this agreement,
this aggrement is effective as of the date listed below.
Terms and Conditions
Customer agrees to abide by the Valley Computer Services acceptable use
policy as provided on the Valley Computer Services web site at
http://www.vcoms.net/acceptable.html. Breech of these acceptable uses
may result in termination of customer's DSL service and early termination
penalties, as outlined below.
Early Termination
Customer agrees to pay Valley Computer Services the lessor of $250.00
or remaining balance of the 12 month contract term for cancellation of
customer's Valley Computer Services DSL service before the end of the
term outlined above. Termination fees will be assessed for early
cancellation for any reason, including but not limited to customer request,
non-payment, or breach of Valley Computer Services acceptable use policy.
Termination fees are in addition to any existing liabilities for services
and equipment provided by Valley Computer Services.
