DSL MONTHLY AGREEMENT

This agreement governs DSL service provided to customers by Valley Computer Services.

Description of Charges
Customer agrees to pay $__________ per month for DSL service provided to customer by Valley Computer Services.

This signed contract must be returned with the first month service fee and any activation fees before the order will be placed.

Any equipment provided by Valley Computer Services remains the property of Valley Computer Services and must be returned at the end of service.

Customer may upgrade to a higher rate DSL plan during the term of this contract for a one-time fee of $60.00.

Customer may switch DSL service to a new phone number or address during the term of this contract for a one-time fee of $60.00.

For new DSL accounts, this agreement is effective when the DSL service is installed. For DSL accounts existing prior to this agreement, this aggrement is effective as of the date listed below.

Terms and Conditions
Customer agrees to abide by the Valley Computer Services acceptable use policy as provided on the Valley Computer Services web site at http://www.vcoms.net/acceptable.html. Breech of these acceptable uses or non-payment may result in termination of customer's DSL service.
